Liver-Kidney Microsome-1 Ab (LKM 1)

Liver-Kidney Microsome-1 Ab (LKM 1)

Diagnosing the Body’s Attack on Its Own Liver Cells

The LKM Antibody Test detects the presence of autoantibodies (specifically Anti-LKM-1) that mistakenly target enzymes within your own liver and kidney cells. This test is a critical tool for diagnosing Autoimmune Hepatitis Type 2 (AIH-2).

In Autoimmune Hepatitis, your immune system—which is supposed to fight outside invaders—sees your own healthy liver tissue as a threat and launches a sustained attack. The LKM antibody acts like a highly specific "Self-Destruct" signal for the liver.

Why Do You Need This Test?

The LKM test is typically ordered by a Hepatologist (liver specialist) or Gastroenterologist when routine testing shows high liver enzymes (ALT/AST) and other common causes (Viral Hepatitis, Alcohol, Fatty Liver) have been ruled out. It is particularly relevant for:

  • Differentiating AIH Types: LKM-1 is the primary marker for Autoimmune Hepatitis Type 2, which most commonly affects children and adolescents.
  • Investigating Unexplained Hepatitis: When a patient presents with symptoms like fatigue, jaundice, and joint pain, but is negative for Hep A, B, and C.
  • Monitoring Treatment: The level of LKM antibodies may be monitored to see if immunosuppressive medication is effectively calming the immune system.

Frequently asked questions on " Liver-Kidney Microsome-1 Ab (LKM 1) "

Autoimmune Hepatitis (AIH) is classified into types based on these antibodies: AIH Type 1: Uses ANA (Antinuclear Antibody) and/or SMA (Smooth Muscle Antibody). This is the most common form. AIH Type 2: Uses LKM Antibody. Ordering all three ensures your specialist finds the right type, which dictates the long-term management strategy.

No, a positive LKM is serious. Autoimmune Hepatitis can be "silent" for years, but the damage (scarring and fibrosis) is happening continuously. A positive LKM result, especially with elevated liver enzymes, is a green light for treatment with immunosuppressants to prevent irreversible liver damage (cirrhosis or liver failure).
